A discharge planenr will, if needed, assist you and your family in making arrangements for care after your stay in the hospital. The Case Managers can help in selecting a convalescent facility, arranging for home health care services, or locating durable medical equipment. There is also a social worker who can help identify support services that may be helpful to you once you go home. If a social worker hasn’t already met with you, please ask your nurse to notify the Social Worker if you wish their assistance.
